The relationship between Alzheimer's disease and insomnia: an integrative review

Abstract

The phenomenon of changes in sleep patterns is more frequent in the more advanced stages of Alzheimer's Disease (AD). These changes become more noticeable, although they can also appear in the early stages of the disease. Therefore, the following research aims to develop an integrative review regarding the relationship between Alzheimer's disease and insomnia. This is an integrative review. The search resulted in 27 articles in SciELO, 397 in PubMed and 76 articles in the LILACS database. Obeying the inclusion and exclusion criteria, few answered the guiding question, with 1 article being considered in SciELO, 5 in PubMed and 3 in LILACS. Therefore, 9 articles remained for analysis in this review. The results indicate that Trazodone, together with orexin receptor antagonists, stands out as having the most robust scientific basis supporting its effectiveness in treating insomnia and long-term cognitive function. On the other hand, the evidence on the benefits of Melatonin and Ramelteon is mixed and consequently somewhat limited. In addition antipsychotics, benzodiazepines, and Z-drugs should be avoided due to their significant adverse effects, particularly sedation, which is a common side effect of all three drug classes. Although Trazodone and Orexin receptor antagonists are considered potential treatment options for insomnia in these patients, it is important to note that more comprehensive studies with greater resources and data are needed to effectively establish their use in clinical practice.
