Nursing and Advance Directive of Will: an integrative review

Introduction: Population aging, along with scientific and technological advancements, has brought about challenges in healthcare, such as high costs and prolonged suffering in hospital settings. Advance Directives (AD) have emerged as essential tools to uphold patient autonomy and dignity. Objective: To identify scientific literature addressing the role of nursing in the Advance Directives. Method: An integrative review guided by the question: “What is the role of nurses in ensuring the right to die through the effective application of Advance Directives for patients at imminent risk of death?” Articles published over the last ten years (2014– 2024) were reviewed across databases including SCIELO, LILACS, BDENF, MEDLINE, CINAHL, SCOPUS, Web of Science, and Science Direct. Results: A total of 750 articles were identified in the databases. In addition, 2 articles were added to the study based on a specialist's recommendation, and following the screening and eligibility stages, 6 were chosen for analysis.Five thematic categories emerged: Lack of preparation and awareness among nurses, Absence of legal support, Patient autonomy, Family resistance, and Religious and spiritual influences on end-of-life decisions. Conclusion: Advance Directives play a key role in promote patient autonomy, particularly in end-of-life situations. Nursing has a critical role in this process, acting as a bridge between patients, families, and healthcare professionals. However, significant challenges remain, including nurses’ lack of preparation, insufficient knowledge, inadequate legal support, and cultural and religious barriers.
