Diagnosis of syphilis during the period of pregnancy: an integrative review

In Brazil, gestational syphilis has shown a significant increase in recent years . In this way, the following study aims to highlight the importance of early and accurate diagnosis of syphilis during pregnancy to promote maternal and fetal health. This is an integrative review. The search resulted in 10 articles in SciELO, 219 in PubMed and 47 articles in the database LILACS. According to the inclusion and exclusion criteria, few answered the guiding question, and 1 article was considered in SciELO, 4 in PubMed and 3 in LILACS. This left 8 articles for analysis in this review. The results show that the diagnosis of syphilis in adults can be carried out by means of direct tests, in which the identification of T. pallidum is sought in samples obtained directly from the lesions, such as dark field examination and direct research using specific staining. In addition, immunological tests are widely used and are subdivided into two categories: treponemal and non-treponemal. Evidence from systematic reviews and meta-analyses indicates an increased risk of these outcomes in pregnant women diagnosed with syphilis, especially those who have not received treatment or whose therapeutic management has been inadequate. In this way, inequality in access to diagnosis and treatment, especially in regions with lower socioeconomic development, reinforces the urgency of public policies that guarantee a more equitable and effective approach to combating the infection.
