Challenges of Hospital Clowning: a suggestive analysis

In the hospital environment, characterized by power relations that may affect patient autonomy, the hospital clown emerges as an integrative humanization practice, promoting welcoming interactions, emotional well-being, potential physiological benefits, and reduced length of hospital stay. This study aimed to analyze the challenges faced by hospital clown groups with more than two decades of continuous activity, identifying factors that contribute to their institutional consolidation and expansion. It is a qualitative, descriptive, and suggestive study based on semi-structured interviews conducted in 2021 via Google Meet with coordinators of these groups. Data were analyzed using thematic Content Analysis according to Bardin. The findings indicate that the sustainability of these initiatives depends on the establishment of solid institutional relationships and strategic alignment with multidisciplinary healthcare teams. Persistent challenges include limited understanding of the hospital clown’s function within the care context and financial instability that compromises the continuity of activities. It is concluded that group coordinators must understand hospital discipline, institutional interests, and the symbolic and relational impacts of their work within the healthcare continuum in order to strengthen the legitimacy and integration of hospital clowning within institutional settings.
