The role of nurses in therapeutic adherence in patients with type 1 and type 2 Diabetes Mellitus in Primary Health Care: an integrative review

Introduction: Diabetes is one of the most relevant chronic conditions today; it is a complex metabolic disease characterized by persistent hyperglycemia resulting from dysfunctions in insulin secretion or action. Objective: To analyze the role of nurses in therapeutic adherence in patients with type 1 and type 2 Diabetes Mellitus in Primary Health Care. Method: This is an integrative review, whose bibliographic research was conducted in April 2025, through searches in the following databases: Latin American and Caribbean Health Sciences Literature (LILACS), Nursing Database (BDENF), Scientific Electronic Library Online (SciELO), Medical Literature and Retrieval System Online (MEDLINE), SCIELO and PubMed. For the selection of articles, the following Health Sciences Descriptors (DeCS) were used: “Role of the nurse”, “Diabetes Mellitus”, “Primary Care” and “Therapeutic Adherence” and terms from Medical Subject Headings (MeSH): “Role, Nurse's”, “Therapeutic Adherence and Compliance”, “Diabetes Mellitus” and “Care, Primary Health” and their synonyms articulated with the Boolean operators “AND” and “OR”. Results: 3,559 articles indexed in the databases were identified; of these, 1,380 were excluded due to duplication, leaving 2,179 for screening through title reading and finally 50 results for full-text reading. Fifteen works were selected for this review. Conclusion: Nurses play an essential role in therapeutic adherence for patients with type 1 and type 2 Diabetes Mellitus in Primary Health Care, acting through health education, continuous monitoring, and building relationships with users. However, challenges such as work overload, deficiencies in professional training, and socioeconomic barriers limit the effectiveness of these actions.
