Factors Associated with the Interruption of Chemotherapy in Patients Treated at a High-Complexity Oncology Care Unit in Brasília

The discontinuation of chemotherapy compromises its efficacy and reduces the life expectancy of cancer patients. This study aimed to identify the factors associated with the interruption of chemotherapy sessions in patients treated at the Oncology Outpatient Clinic of the Hospital Regional de Taguatinga (HRT), a High-Complexity Oncology Care Unit in Brasília, Brazil. This is a cross-sectional observational study conducted between March and September 2024, involving adult patients diagnosed with cancer. Data collection was carried out through a review of electronic medical records and an analysis of scheduling records. Of the 4,311 scheduled sessions, 979 (22.7%) were canceled or rescheduled. The majority of patients were female (71.8%), with a mean age of 56 years. Breast cancer was the neoplasm most associated with interruptions (30.6%), followed by colorectal cancer (16.0%) and cervical cancer (12.9%). Clinical stage assessment revealed a predominance of advanced diagnoses, with 36.2% in stage III and 34.6% in stage IV, characterized by metastatic disease. Clinical factors accounted for 59.1% of suspensions, with hematologic toxicity (25.6%) and infections (18.1%) being the most prominent causes. Personal factors represented 20.9%, with unexplained patient absence as the primary reason (54.6%). Institutional factors accounted for 16.7%, mainly due to a lack of infusion time availability (35.0%) and scheduling errors (19.0%). The findings highlight the need to optimize hospital management, improve protocols for managing toxicities and infections, and strengthen educational and psychosocial support for patients, aiming to minimize interruptions and optimize clinical outcomes.
