Abortion as a Public Health Issue: Profile of Hospitalizations for Abortion in Northeastern Brazil from 2011 to 2020

INTRODUCTION: Abortion is a serious public health issue, with an average of 73.3 million induced abortions worldwide per year (WHO, 2020; BEARAK et al., 2020). OBJECTIVE: To describe the profile of abortion hospitalizations in Brazil, specifically in the northeastern region, from 2011 to 2020, based on official health information systems. METHODOLOGY: This is an ecological, descriptive, and cross-sectional study using population-based data to outline the profile of abortion hospitalizations in the northeastern region of Brazil. Publicly available data from the Brazilian health information systems (SIH/SUS) will be analyzed according to the following variables (when available): northeastern states, sociodemographic characteristics (age and race/ethnicity), age groups, classification of abortion (legal/ spontaneous/ other types/ unspecified), year of occurrence, and costs of abortion hospitalizations. RESULTS: From 2011 to 2020, there were 596,953 abortion hospitalizations in the Northeast, corresponding to an average annual rate of 23.03%. When analyzing the age groups, all of them started with lower recorded rates in 2011 (15-19 years: 0.14%, 20-29 years: 1.25%, 30-39 years: 0.75%, and 40-49 years: 0.14%) compared to other years. The evaluation by race/ethnicity indicated 40.16% of cases with missing information in the last ten years. The mortality rate of women who had spontaneous abortions was higher among brown-skinned women (30.38%). CONCLUSION: This study successfully described the profile of abortion hospitalizations in the northeastern region and evaluated the trends over the past decade. Therefore, it highlights the need for further research on this topic to obtain more accurate portraits of abortion hospitalizations in the Northeast, including the causes of unsafe abortion hospitalizations, in order to promote the development of public policies to address this social issue surrounding abortion.
