Nursing role in the care of patients with stroke at different levels of health care

This study aimed to analyze the role of nursing in the care of patients with stroke (cerebrovascular accident) across different levels of health care, highlighting professional competencies, clinical protocols, educational strategies, and challenges faced. This is a literature review with a qualitative approach, based on the analysis of scientific publications obtained from the BVS, SciELO, MEDLINE/PubMed, LILACS, and Scopus databases, as well as official documents from the Ministry of Health. Studies that directly addressed the nurse’s role in assisting stroke patients in hospital, home, and primary care settings were included. The analysis revealed that nursing plays a central role in all stages of stroke patient care. In emergency and urgent care contexts, nurses perform early recognition of clinical signs, risk classification, application of standardized scales (such as NIHSS and Cincinnati), and implementation of thrombolysis protocols. In hospital care, neurological and hemodynamic monitoring, prevention of secondary complications, early rehabilitation, and humanization of care stand out. In primary care, home follow-up, health education, and family support strengthen the continuity of care, preventing recurrences and promoting quality of life. However, the practice faces challenges such as fragmentation of the emergency network, shortage of hospital beds, lack of standardized protocols, and work overload, which hinder the provision of quality care. It was observed that nursing practice is essential for improving prognosis, reducing morbidity and mortality, and ensuring comprehensive and safe care for stroke patients. Integration among the different levels of care, implementation of evidence-based protocols, and health education are essential strategies to optimize clinical outcomes and the quality of care.
